*Alcohol service requires approval of the Campus Activities office. THREE weeks advanced notice is required for all Locations,  to acquire an off site catering permit. All alcohol service must be provided by Matthew’s Mug.

By entering the initials and submitting this request the requestor agrees on behalf of the organization to adhere to all college regulations and policies.  The requestor accepts responsibility for payment of any and all expenses associated with provisions of security, firewatch, set-up/breakdown, repair or replacement and returning the facility to its original condition at the completion of the activity.
